Tibial tuberosity avulsion

Presentation

Was playing with other dogs the previous day. Today weight bearing lame on the left pelvic limb.

Patient Data

Age: 5 m
Gender: Female
Category: Domestic dog
Organism: Bernese mountain dog

Stifle

x-ray

On radiographs of the left stifle, there is mild widening of the tibial apophyseal physis with a small fragment visible at the distal portion. The patellar ligament appears normal and there is no stifle effusion present. The remainder of the pelvic limb appears normal. Comparison views of the right stifle were obtained, and confirm the widening of the left tibial apophyseal physis. On flexed views of the left pelvic limb, the widening is exacerbated compared to the right which remains static.

Case Discussion

Tibial tuberosity avulsion fracture. The injury was managed conservatively and healed well.

In this case, the displacement of the tibial tuberosity was minimal. Using the opposite limb for comparison, and increasing the tension on the patellar tendon on the flexed view to further displace the fracture provided additional evidence that the fracture was present.
