Salter-Harris Fracture

Case contributed by Allison L Zwingenberger
Diagnosis certain

Presentation

Presents acutely non-weight bearing on the left thoracic limb.

Patient Data

Age: 3 m
Gender: Female
Category: Domestic cat
Organism: Domestic shorthaired cat

Elbow

x-ray

Right elbow: Normal for comparison.

Left elbow: There is a type 3 Salter-Harris fracture through the lateral distal humeral physis and epiphysis with mild cranial displacement of the humeral condyle in relation to the proximal humeral fracture fragment. There is moderate soft tissue swelling surrounding the distal aspect of the left humerus.

Elbow - 3-week recheck

x-ray

The previously identified fractures through the middle of the distal epiphysis of the humerus and extending laterally through the condyle is again identified and there is smooth bony remodeling around this region. This fragment has not displaced and has not changed position in the recheck interim.

Case Discussion

This fracture was treated conservatively because healing occurs quickly in young animals, and the small size of the bones makes internal fixation challenging. In three weeks the fracture was stabilized and healing well.

Salter-Harris fractures have defined configurations that are classified with regards to the involvement of the physis, metaphysis, and epiphysis.
