Case contributed by Allison L Zwingenberger
Diagnosis certain

Presentation

Vomiting for the past three months with acute worsening last night. Weight loss in last 6 months.

Patient Data

Age: 11 years
Gender: Castrated male
Category: Domestic cat
Organism: Persian cat

Abdomen

x-ray

In the abdomen, there is adequate serosal detail. The kidneys are enlarged bilaterally, best seen on the ventrodorsal projection. The kidneys have a mildly irregular border. Nephroliths are present bilaterally with irregular borders. The colon is moderately feces and gas filled.

Impression:

Bilateral renomegaly is likely due to polycystic kidney disease with degenerative change.

Urinary tract

ultrasound

Both kidneys contain multiple mostly anechoic and mildly echogenic cysts. Some cysts have sedimenting echogenic content. The renal architecture has poor corticomedullary distinction, and there are small calculi in the renal pelvis.

Case Discussion

Bilateral renomegaly indicates a systemic disorder such as degenerative or inflammatory. In Persian cats, polycystic kidney disease is common. The irregular borders indicate nodules or cysts causing the kidney enlargement. Ultrasound can evaluate the renal architecture in detail.
