Presentation

Presents after being hit by car earlier today. No palpable bladder.

Patient Data

Age: 12 years
Gender: Castrated male
Category: Domestic dog
Organism: Jack Russell terrier

There are multiple pelvic fractures including the sacrum, right acetabulum, bilateral ischium, pubis. The right coxofemoral joint and left sacroiliac joint are luxated. The urinary bladder is not visible, however the peritoneal detail is good and no effusion is present. There is a fragment of bone cranial to the pubis and the body wall is not continuous with the pubis.

Multiple pelvic fractures and prepubic tendon avulsion.

Cystourethrogram

A cystourethrogram was performed to evaluate the position and integrity of the urethra and bladder. Contrast medium was injected into the urethra. There is no evidence of leakage of the contrast material from the urethra or urinary bladder. The urinary bladder is located inside the abdomen. The cranioventral portion of the bladder seems to be in a ventral position compared to its normal anatomical position, possibly due to the prepubic tendon rupture.

Intact bladder and urethra, no bladder herniation.

Case Discussion

Trauma to the pelvis can include fractures as well as rupture of the urinary tract and body wall hernias. Since the bladder was empty at presentation, confirmation of an intact bladder was performed for prognosis and surgical planning. The peritoneal detail was good, indicating a uroabdomen was unlikely, however it was important to rule out this differential diagnosis.

A screw was placed through the left sacroiliac luxation to stabilize it. A right femoral head and neck osteotomy was performed as the acetabular fracture could not be repaired. The prepubic tendon rupture was not repaired as the bladder was not herniated.
