Presentation

Presents with intermittent left thoracic limb lameness.

RIGHT SHOULDER: There is flattening of the caudal aspect of the humeral head with a mildly irregular crescent-shaped defect in the subchondral bone of the caudal aspect of the humeral head which is larger than on the left. There is sclerosis and heterogeneity of the caudal humeral head. A small, linear osseous body is seen caudal and distal to the right humeral head.

LEFT SHOULDER: There is flattening of the caudal aspect of the right humeral head with a mildly irregular concave defect in the subchondral bone in this location involving approximately 1/4 of the margin of the caudal humeral head. There is mild associated sclerosis of the caudal humeral head. There are multiple open physes consistent with skeletal immaturity.

Case Discussion

Osteochondritis dissecans is often bilateral, and it is prudent to image both joints. It occurs in characteristic locations in joints, typically the point of highest load under weight bearing.
