Presentation
Presents with two-month history of fever and lameness.
Patient Data
There are radiolucent bands in the metaphyses of multiple long bones parallel to the physes. The soft tissues surrounding the metaphyses are thickened, and there is periosteal reaction caudal to the left and right distal ulna within the soft tissues. There is smooth periosteal thickening on the right proximal radius.
Metaphyseal osteopathy.
Case Discussion
Metaphyseal osteopathy (also termed hypertrophic osteodystrophy) is characterized by necrosis of the metaphyseal bone parallel to the physis. Histologically, there is suppurative inflammation and disappearance of the osteoblasts. Radiographically, lesions are bilaterally symmetric, and affect the limbs proximal to the tarsus and carpus. Some animals will develop an intense periosteal reaction around the metaphysis and in the soft tissues, leading to severe thickening. This case has both periosteal reaction originating from the metaphysis, and the type that occurs disconnected from the bone within the soft tissues. The metaphyseal necrosis appears as a second, though irregular radiolucent line parallel to the physis.
