Intestinal large cell lymphoma

Presentation

Presents with anorexia and intermittent diarrhea for the last month.

Thorax

x-ray

There is a broad based soft tissue opacity dorsal to the third sternebra on the lateral projections consistent with an enlarged sternal lymph node. The pulmonary parenchyma appears normal. The cardiovascular structures are also normal. Within the abdomen, the liver is enlarged and rounded, and there is increased soft tissue opacity in the peritoneum.

Case Discussion

Probable metastatic lymphoma. Jejunal large granular cell lymphoma was diagnosed by cytology of the peritoneal effusion and the enlarged jejunal lymph nodes. While the lymph node could be reactive, the extremely large size is more consistent with neoplastic infiltration. The sternal lymph node drains the abdomen, and should be a marker of abdominal disease when enlarged.
