Intestinal foreign body

Presentation

Presents with three-day history of lethargy, anorexia and two bouts of vomiting.

Abdomen

x-ray

Within the abdomen, there is marked gas and fluid distention of multiple loops of small intestine as well as other loops of small intestine that appear within normal limits. In the caudoventral abdomen, there is a focal area of regularly spaced radiolucencies within the small intestine that represent a foreign body such as a corn cob. There is a large volume of gas within the colon. There is mild loss of serosal detail. The splenic margin is mildly irregular with a rounded nodule appearing to protrude from the margin of the spleen, noted on the right lateral projection. Incidental metal fragments are present overlying the pelvis.

There are multiple fluid distended loops of small intestine, one of which contains foreign material with acoustic shadowing. The small intestinal wall is thickened in this region with surrounding hyperechoic mesentery and a small amount of free fluid. There is mild mesenteric lymphadenopathy. Two masses are present within the spleen.

Mechanical foreign body obstruction with local enteritis and possible peritonitis. Probable reactive mesenteric lymphadenopathy. Splenic nodules may be lymphoid hyperplasia or neoplasia.

Case Discussion

Two corn cobs were removed via enterotomy and a splenectomy was also performed. The splenic nodules were diagnosed as lymphoid hyperplasia and extramedullary hematopoeisis on histopathology. The dog had septic peritonitis at the time of surgery.
