Intestinal foreign body

Case contributed by Allison L Zwingenberger
Diagnosis certain

Presentation

Presented with acute onset of vomiting and anorexia for 12 hours.

Patient Data

Age: 8 months
Gender: Castrated male
Category: Domestic dog
Organism: Chihuahua superbreed

Abdomen

x-ray

There is a large, oval, mineral opacity structure in the gastrointestinal tract. The stone is positioned medial to the cecum on the ventrodorsal projection. The cecum and descending colon are gas filled. There are several mildly distended, gas filled small intestinal loops in the left cranial abdomen. Mildly distended fluid filled loops are present on the lateral projections. The remainder of the abdominal organs are normal.

Impression:

Intestinal foreign body causing mechanical obstruction. The position of the stone may be in the distal jejunum or ileum.

Abdomen +2 hours

x-ray

A lateral radiograph taken less than two hours after the initial radiographs shows the stone in the rectum. The small intestinal dilation has resolved. The cecum is gas filled. Passage through the ileocolic junction likely occurred between the radiographs.

Case Discussion

The stone was obstructive on the initial set of radiographs. With time and medical management, it is possible for foreign bodies to pass into the colon and resolve the mechanical obstruction. While this was a remarkably short time interval for that to occur, it is worth considering additional radiographs prior to surgery to ensure the obstruction is still present with longer time intervals.
