Presentation

Presented with vomiting, weight loss, and suspected IBD after exploratory laparotomy and biopsy two months previously.

Age: 10 years
Gender: Castrated male
Category: Domestic cat

Abdomen

x-ray

There are multiple loops of markedly distended small intestine. The colon appears to be smaller in diameter than the majority of the intestinal loops. The most markedly distended loop is located in the cranial abdomen and contains granular material as well as small mineral opacity particles. The abdomen is distended due to the mass effect of the enlarged intestinal loops. There is mineralization in the kidneys seen on the lateral projection.

Case Discussion

Annular adenocarcinoma of the distal ileum caused chronic partial obstruction.

The stricture and proximal dilated bowel were identified on ultrasound examination prior to surgery. The low volume mass and fibrous nature of these tumors makes them difficult to identify. The secondary signs of chronic partial obstruction are an indication of the mechanical obstruction. The stricture at the obstructed site, next to the largest segment of intestine, indicates a mural lesion.
