Hypertrophic osteodystrophy

Case contributed by Allison L Zwingenberger

Presentation

Presents with reluctance to walk.

Patient Data

Age: 5 months
Gender: Male
Category: Domestic dog
Organism: Mastiff dog

Skeleton

x-ray

Left thoracic limb: There is sclerosis and irregular radiolucency of the metaphysis of the distal radius and ulna. There is increased intramedullary opacity of the proximal radius and ulna.

Right thoracic limb: The changes in the right radius and ulna are similar to those observed in the left antebrachium.

Pelvis: There is mild incongruity of the right coxofemoral joint and moderate incongruity of the left coxofemoral joint with poor coverage of the left femoral head by the left acetabulum. There is mild periarticular remodeling at the cranial aspect of the left acetabulum. A mild increase in intracapsular soft tissue opacity is present associated with both stifle joints. A crescent-shaped, radiolucent defect is present in the subchondral bone of the lateral condyles of the right and left femur. There is a subtle increase in intramedullary opacity of both femurs.

Sclerosis and irregular radiolucency of multiple metaphyses consistent with hypertrophic osteodystrophy. Bilateral hip dysplasia worse on the left with mild left coxofemoral joint osteoarthrosis. Bilateral stifle joint effusion with subchondral bone defects in the lateral condyles of the femurs consistent with osteochondrosis. Suspect panosteitis.

Case Discussion

Young animals can have multiple forms of developmental orthopedic disease concurrently. In this type of case it is important to take additional radiographs if needed, and to be alert for musculoskeletal abnormalities.
