Diagnosis certain

Presentation

Presents with shallow, labored breathing and decreased appetite.

Patient Data

Age: 5 years
Gender: Spayed female
Category: Domestic cat
Organism: Domestic shorthaired cat

Thorax

x-ray

There is moderate generalized cardiomegaly with a "valentine" shape on the DV projection. The pulmonary parenchyma and the vasculature size is unremarkable. These findings are consistent with cardiomyopathy. Within the viewable abdomen, there are mineral densities within the cranial dorsal abdomen. These are likely degenerative change of the adrenal glands.

Case Discussion

Echocardiography confirmed severe hypertrophic cardiomyopathy with left sided concentric hypertrophy, severe systolic anterior motion (SAM) of the mitral valve, and dynamic right ventricular outflow tract obstruction. The cat had previous episodes of heart failure, however this was not detected on the current images.

Cardiomyopathy in cats results in a base-wide heart on the dorsoventral or ventrodorsal projection, which is easier to detect than the generalized cardiomegaly on the lateral projection.
