Presentation

Evaluation for chronic renal failure.

Patient Data

Age: 9 years
Gender: Castrated male
Category: Domestic cat
Organism: Domestic shorthaired cat

Abdomen

x-ray

The liver and gastrointestinal tract appear normal. The left kidney is small with an irregular border and central calculi in the pelvis. The right kidney is enlarged and has a flattened cranial pole on the ventrodorsal projection. Calculi are present in the renal pelvis.

Bilateral chronic renal change and calculi. Right renal enlargement may be due to compensatory hypertrophy or hydronephrosis.

Ultrasound imaging showed an enlarged right kidney with marked pelvic dilation and poorly defined architecture. The left kidney was small with mild pelvic dilation.

Case Discussion

Right renal hydronephrosis secondary to right ureteral obstruction (mucus plug) and moderate hypertrophy (histopathology).

On radiographs, an enlarged kidney combined with a small kidney may indicate compensatory hypertrophy. However, obstruction of the ureter and enlargement of the renal pelvis can also contribute to increased size. This case had a combination of both disorders.
