Presentation

Coughing and increased respiratory effort for three weeks. Appetite decreasing for two weeks.

Patient Data

Age: 6 years
Gender: Castrated male
Category: Domestic dog
Organism: Shih tzu dog

Thorax

x-ray

The cardiac silhouette is enlarged, with predominantly right sided enlargement. The pulmonary arteries are enlarged and tortuous, with abrupt tapering in the periphery. The main pulmonary artery is enlarged on the dorsoventral projection. There is a diffuse bronchointerstitial pattern throughout the lungs, most prominent caudodorsally. The peritoneal detail is decreased.

Case Discussion

Heartworm disease with pneumonitis and peritoneal effusion secondary to caval syndrome.

9 heartworms were retrieved from the right atrium via a jugular approach. The heartworm disease was class 4, including right heart failure, anemia, hemoglobinuria, and lethargy. The pulmonary hypertension and pulmonary changes improved over the next 1.5 years after treatment.
