Presentation
Presents with an acute increase in respiratory effort.
Patient Data
The cardiac silhouette is moderately enlarged, which is most visible on the dorsoventral projection. The left atrium is moderately enlarged and the trachea is elevated. There is an alveolar pulmonary pattern in the right caudal lung lobe with visible air bronchograms. The cranial lung lobes and pulmonary vasculature appear normal. There is moderate gas distension of the stomach with some laminar soft tissue foreign material.
After initiating diuretic therapy, enalapril, and pimobendan, the pulmonary edema resolved. The enlarged left atrium is now visible on the lateral projection.
Case Discussion
Cardiomegaly with left sided heart failure and pulmonary edema, most likely secondary to degenerative valvular disease. Gastric foreign material which did not cause clinical signs.
Echocardiography showed left sided cardiac failure due to myxomatous mitral and tricuspid valve degeneration. Medical management controlled the heart failure.
