Gastric dilation and volvulus

Presentation

The dog has been boarding at the referring veterinarian for 9 days. Presents with bloat and distress after evening meal.

Age: 9 years
Gender: Spayed female
Category: Domestic dog
Organism: Labrador retriever

Abdomen

x-ray

The stomach is markedly distended with gas and is causing a mass effect, displacing the spleen and small intestine caudally. The pylorus is gas filled and is malpositioned dorsally relative to the fundus. There is granular mineral material within the stomach. The caudal vena cava is small.

Gastric dilation and volvulus. Gastric foreign material, likely incidental. Possible hypovolemic shock.

Case Discussion

A right lateral projection is often all that is needed to make the diagnosis of GDV. Additional views may be taken to confirm the malposition of the compartments if the patient is stable. Often, as in this case, there is hypovolemic shock and rapid resuscitation is needed.

Following treatment for metabolic acidosis and shock, abdominal surgery was performed to replace the stomach in a normal position and perform a gastropexy. No necrosis of the stomach was found.
