Presentation
Bilateral pelvic limb lameness, progressing over the last three months.
Patient Data
There is bilateral tarsal effusion affecting all joints, and particularly within the tibiotarsal joint. There is extensive periarticular osteophyte formation with relatively smooth margination. The tarsal bones appear heterogeneous with areas of lysis.
Impression:
Marked erosive polyarthritis.
Case Discussion
Joint capsule biopsy revealed multifocal, moderate proliferative and suppurative synovitis, and joint fluid had marked suppurative inflammation. Differential diagnoses included infectious and autoimmune polyarthritis, however no organisms were detected. The patient was started on predinisolone and cyclosporine, and improved greatly over the next few months with increased activity and improvement of the lameness. The radiographic signs were consistent with erosive polyarthritis.
