Presenting for progressive carpal hyperextension and decreased activity.

Age: 9 years
Gender: Spayed female
Category: Domestic dog

There is soft tissue swelling surrounding both carpi. The carpal bones are misshapen and there are multiple subchondral radiolucent regions. The right radial carpal joint is subluxated. Several interphalangeal and metacarpophalangeal joints are similarly subluxated. There is periosteal reaction on the distal radius, dorsal carpus, and accessory carpal bone.

Case Discussion

Erosive polyarthritis with secondary osteoarthritis and joint instablility. The subchondral defects are a result of the inflammatory process, and predispose the joint to secondary osteoarthritis and laxity of the surrounding tendons and soft tissues.

The disease was medically managed and the dog was in remission. Euthanasia was performed three years later due to decreasing quality of life. Histopathology of the carpal and tarsal joints showed severe, chronic arthritis with synovial osteochondromatosis, articular cartilage eburnation and necrosis.
