Presentation

Presenting with episodes of coughing and weight loss for the last three months.

Thorax

x-ray

On radiographs of the thorax, there is a diffuse, bronchointerstitial pattern with a nodular component. The lung lobes are rounded and retracted, with increased opacity in the pleural space. The cardiac silhouette is somewhat obscured but appears normal in size.

Case Discussion

Ultrasound guided fine needle aspirate of a pulmonary nodule was performed, and adenocarcinoma with inflammatory and neoplastic effusion was diagnosed on cytology.

Diffuse bronchointerstitial and nodular pattern may be caused by chronic lower airway disease, inflammatory disease, or neoplasia. The pleural component suggests effusion or soft tissue in the pleural space.
