Presentation
Intermittent right thoracic and left pelvic limb lameness, mild fever, and increased respiratory rate. Dog was adopted from Mexico.
Patient Data
There is a diffuse miliary nodular pattern in the pulmonary parenchyma. There is mildly increased opacity dorsal to the carina in the region of the tracheobronchial lymph nodes. There is stippled irregular periosteal reaction and multifocal lysis of the wing and spine of the scapula seen on the lateral projections.
Radiographs of the right thoracic limb were obtained. There is a similar stippled lesion with irregular periosteal reaction and regions of lysis in the mid diaphysis of the right radius. Oblique projections of the scapula were obtained and confirmed the previous finding of aggressive lesions.
Multiple nodules are present throughout the lung tissue, and are visible on this cut section.
Case Discussion
The pulmonary and aggressive osseous changes are consistent with an infectious disorder. The origin of this dog from the Southwest region of North America suggests that a fungal disease such as coccidioidomycosis is most likely.
Further diagnostics revealed bilateral ocular lesions, and additional sites of bone involvement.
The coccidiodes titer was positive and treatment was attempted with antifungals, howerver was unsuccessful. At necropsy there were yeast spherules in all tissues except the gastrointestinal tract, including the brain.
