Presentation

Presented with 5-week history of increasing left hind limb lameness and lethargy.

Patient Data

Age: 3 years
Gender: Castrated male
Category: Domestic dog
Organism: Shih tzu dog

Pelvis

x-ray

There is a mixed lytic and productive bone lesion on the wing of the left ilium, extending to the cranial acetabulum. The lesion is seen best on the v/d projection.

Thorax

x-ray

There is thickening and irregular periosteal reaction of the right scapular spine.

Case Discussion

Differential diagnoses for polyostotic aggressive bone lesions include osteomyelitis (bacterial, fungal) and metastatic neoplasia.  

Ultrasound-guided fine-needle aspirates of the bone and sublumbar lymph node were taken and showed inflammatory change. A coccidiodomycosis titer was high at 1:32, indicating a severe infection.
