Avulsion fracture - suspensory ligament

Presentation

Presents for a bone scan of forelimbs. Raced approximately a week ago and came in first/second. The next day after galloping he was found to be 3/5 lame on his RF, nothing remarkable was found. The next day the lameness was markedly improved, radiographs were clean. He was rested for several days and when started back to work was still lame on his left front.

Baseline Study

Nuclear medicine

There is marked, focal radiopharmaceutical uptake at the palmaroproximal aspect of the right third metacarpal bone on midline. Mild, focal increased uptake is observed in the dorsal mid-diaphysis of the left third metacarpal bone. Mild, diffuse increased radiopharmaceutical uptake is present in the distal left radius and bilaterally in the metacarpophalangeal joints.

R MetaCarpus (1 day follow-up)

x-ray

A lucent lesion is present in the medial to mid portion of the proximal third metacarpal bone in the region of the origin of the suspensory ligament. Within the lucency there is a thin opacity that can be seen displaced from the apparent bone on the medial oblique projection.

Case Discussion

Suspensory ligament desmitis with avulsion fracture. The chronic desmitis and acute fracture both contribute to the increased radiopharmaceutical uptake on the bone scan. Radiographs are used to confirm the lesion.
