Presentation

Presented with a grade 4/6 holosystolic heart murmur and elevated respiratory rate.

Patient Data

Age: 1 year
Gender: Castrated male
Category: Domestic cat
Organism: Bengal

Thorax

x-ray

The cardiac silhouette is enlarged on all projections. The pulmonary vasculature is distended, and there is patchy interstitial to alveolar pattern in the left and right cranial lung lobes as well as the dependent portion of the right caudal lung lobe.

Case Discussion

Aortic stenosis with heart failure.

Valvular and supravalvular aortic stenosis was diagnosed on echocardiography. The left atrium was enlarged, and there was concentric left ventricular hypertrophy with mitral and aortic valve insufficiency.

In young animals, congenital cardiac disease becomes the primary differential diagnosis in cases of heart enlargement and heart failure. Aortic stenosis is the second most common congenital cardiac anomaly in cats.
