Presentation
Presents with vomiting, lethargy, and anorexia.
Patient Data
The stomach contains a small amount of mineral material. The small intestine has mildly gas filled loops in the caudal abdomen and normal sized loops in the ventral abdomen. The colon is filled with gas and fluid. There is a round soft tissue opacity mass in the ventral abdomen superimposed on the enlarged spleen. There is a rim of fat between the spleen and the majority of the mass. The liver is mildly enlarged with rounded borders.
Mid-abdominal mass may be arising from the spleen, lymph nodes, or mesentery.
The mass visible on the radiographs was 4.8 cm in diameter with multifocal cavitary regions. It was located near the proximal extremity of the spleen but not attached to any organ. An additional hyperechoic lesion was found in the liver, which did not deform the liver capsule.
Case Discussion
A necrotic lipoma was found on exploratory laparotomy arising from the omentum, representing the mass visible on the radiographs. This supports the rim of fat separating the spleen and the mass, as the mass was not splenic in origin. The liver biopsy showed telangiectasia, glycogenosis, lipidosis, and multiple granulomas without a mass lesion.
