Case dicussion

Last revised by Allison L Zwingenberger on 7 Mar 2022

The discussion can summarize the diagnostic findings into a diagnosis, provide the outcome, and report the diagnostic proof of certainty.

The discussion can also be used to add any key points or information that adds to the learner's knowledge of the disease and how it relates to the imaging findings.

Note that you are providing an example of a known case, so extensive differential diagnoses are not necessary. Relevant differential diagnoses can be listed along with how they were excluded. Differential diagnoses make an excellent quiz question to add to the case however.

Case Discussion

This aggressive bone lesion was not surgically resectable, and was treated with a total dose of 48 Gy in 16 3Gy fractions using 10 MV photons. The external mass resolved, and the pelvic bones returned to a near-normal shape and density.

Solitary plasma cell tumors are rare, and are most commonly reported in the axial skeleton.  Plasma cell tumor may also progress to multiple myeloma.

Case Discussion

Pleural effusion with pleural thickening is indicative of pyothorax. The cranial lung lobes are likely atelectic. Mild pneumothorax may be iatrogenic or secondary to pulmonary rupture or abscessation.

Thoracic fluid cytology - Marked purulent septic exudate with evidence of prior and ongoing hemorrhage consistent with pyothorax. No organisms were cultured.
